Patient optimisation and rehabilitation • • • Patient optimisation prior to surgery helps to ensure the best possible outcomes for the patient. Prehabilitation is a process that aims to enhance a patient’s physical and psychological function to support them before, during and after surgery. Prehabilitation and optimisation of comorbidities pre-surgery can: - Reduce the length of stay. - Reduce risk of post-operative complications. - Enhance recovery post-surgery. - Improve nutritional status. - Encourage the patient to quit smoking and drinking alcohol. - Prepare the patient psychologically and emotionally for surgery. - Improve surgical outcomes. - Improve population health. - Improve the patient experience. Criteria-led discharge (CLD) • • • • • Pre-determined criteria that the patient must meet in order to discharge when clinically and socially safe to do so. It ensures that discharge from the hospital is not delayed and safe transition home or another care setting is possible and happens as soon as possible. The decision is made and documented by a senior medical clinician. It involves the care team of the patient agreeing on a set of milestones for the patient to meet. Milestones include a combination of clinical criteria such as having a normal temperature or not needing a drop and social (physical) criteria for example being able to be independent where patient is able to normally live. - These milestones are documented in the medical record. - The medical team works with the patient to ensure that these milestones are met so that the patient can leave the hospital as quickly and as safely as possible.
