Abnormalities of sperm function • Mannose fluorescence assay: acrosomal mannose-ligand receptor activity is correslated with IVF pregnancy rates, as sperm need to recognise the mannose on the zona surface. A capacitated sperm sample is mixed with fluoresced mannose residues, and a known fertile donor sperm is also mixed in a separate sample. The patient’s binding pattern in then compared. • Hemizona Assay: analyses sperms ability to bind to ZP. Human oocytes are bisected and separately mixed with partner’s sperm + control sperm. Hemizona index is calculated by dividing the no of patient sperm bound by the number of control sperm bound. • Acrosomal reaction: % of sperm that undergo the acrosomal reaction in response to induction in measured and compared with a control sample. High FSH and low testosterone levels are indicative of testicular failure. - Most men with oligospermia will have varying degrees if irreversible primary testicular failure. Low FSH and low testosterone -> hypothalamic dysfunction. Normal spermatogenesis requires high T levels, which can’t be replicated with exogenous T. - Many of these men also lack sperm stem cells -> this T replacement won’t help sperm production.
