Genitourinary Examination Component What to do/look for and rationale • Wash hands • Introduce self. • Briefly explain purpose and obtain consent. • Ask about pain before you begin. • Ask for vitals. General inspection • Position patient at 45-degree angle. • Comment on mental alertness/confusion -> signs of azotaemia -> nephrotic and nephritic syndromes. • Skin colour • Rash • Uric flakes present? Hands • Nails -> leukonychia -> albumin loss -> nephrotic syndrome. • Asterixis Arms • AV fistula -> indication of dialysis -> indicated CKD. • Bruising • Scratch marks Face • Conjunctive pallor • Mouth -> mucosa and fetor Neck • JVP • Asculate for carotid artery bruit Heart • Palpate for cardiomegaly. • Asculate for pericardial rub. Lungs • Asculate for pulmonary edema Abdomen • Inspect for signs of kidney transplant or abdominal masses such as polycystic kidney cysts and liver. • Palpate abdomen in the same way as in the gastro exam. • Percuss of ascites. • Examine for signs of enlarged bladder. • Auscultate for abdominal bruits. • Ask to perform a PR examination for the prostate Legs • Peripheral edema • Peripheral pulses.
