RBC production Sites of RBC synthesis • • Site changes with age - First few weeks of Fetal life -> yolk sac - 6 weeks until 6-7 months of Fetal life -> liver and spleen of the foetus are major production sites ✓ Continue to produce cells until about 2 weeks after birth - From 6-7 months of Fetal life -> bone marrow is the most important site for blood production Bone marrow - Infancy ✓ All bone marrow has haemopoietic activity ➢ Assessment of bone marrow insert needle into tibia - Progressive replacement with fat in long bones such as the radius, ulna, tibia and femur - Adults ✓ Bone marrow is confined to central skeleton and proximal femurs and humeri -> composed of 50% fat Erythropoiesis • • Multipotent stem cells - Where all blood cells are derived from - Differentiation into the different blood cell lineages is dependent on cytokines and microenvironment influences Production requirements - Iron (Hb is the largest iron compartment in the body and contains about 2gm of iron - Fe absorption most efficient in the duodenum and becomes progressively less further along the gastrointestinal tract - Haem – a tetrapyrole (4) ring with binding to iron in the ferrous state (Fe2+). Characteristics of a porphyrin. Haem is classified as a metalloporphyrin - Iron in Hb must be in the ferrous state to allow for reversible binding to oxygen - DNA synthesis - Vitamin B12 and Folate for production of DNA Total body content of B12 is 2 to 5mg Folic acid is mainly absorbed by the proximal jejunum. Found in green leafy vegetables
