Drugs acting on ACh system    Hemicholine: decreases synthesis of ACh. o Hemicholinium-3 is an experimental drug which blocks the reuptake of choline by the high affinity transporter ChT at the presynapse. o Reuptake of choline is the rate limiting step in the synthesis of ACh. o Hence, hemicholine decreases the synthesis of ACh. Botulinum toxin: blocks ACh release. o Toxin cleaves SNARE proteins, and so vesicles cannot undergo exocytosis at cell membrane. o Clinical indication: muscle spasms, wrinkles. ACh-esterase inhibitors: increase synaptic transmission. o Clinical indication: chemical warfare, Alzherimer’s disease. Monoamine neurotransmitters  Catecholamines include dopamine, adrenaline, noradrenaline.  Indolamines include serotonin (5-HT). Catecholamine synthesis Dopamine  Used by several neural systems: o Nigrostriatal system: projects from substantia nigra to the caudate nucleus and putamen. o Mesolimbic system: projects from ventral tegmental area to the limbic system (including nucleus accumbens, amygdala and hippocampus). o Mesocortical system: projects from ventral tegmental area to the cortex.   Receptors are metabotropic (G-protein coupled). o D1 receptors are postsynaptic (excitatory). o D2 receptors are pre- and postsynaptic (inhibitory). Termination is by monoamine oxidase (MAO).
