This disease is primarily associated with exposure to asbestos, a mineral once commonly used in various industries due to its heat resistance and insulating properties. Pleural mesothelioma typically has a poor prognosis due to its aggressive nature and limited treatment options, highlighting the importance of early detection and prevention. Causes and Risk Factors 1. Asbestos Exposure: o Inhalation or ingestion of asbestos fibers is the primary cause of pleural mesothelioma. o Occupational exposure is common in industries such as mining, construction, shipbuilding, automotive manufacturing, and asbestos removal. o Secondary exposure can occur through contact with asbestos-contaminated clothing or living in environments with asbestos-containing materials. 2. Other Risk Factors: o Although asbestos exposure is the predominant risk factor, other factors such as radiation exposure, genetic predisposition, and certain viruses may contribute to the development of pleural mesothelioma. Clinical Manifestations The clinical presentation of pleural mesothelioma can vary but often includes:      Respiratory Symptoms: Such as chest pain, shortness of breath, persistent cough, and wheezing. Pleural Effusion: Accumulation of fluid in the pleural space, causing dyspnea and chest discomfort. Weight Loss: Unexplained weight loss may occur due to the systemic effects of cancer. Fatigue: Resulting from the body's response to cancer and associated symptoms. Other Symptoms: Such as fever, night sweats, and hoarseness, although less common. Diagnosis Diagnosis of pleural mesothelioma involves a combination of clinical evaluation, imaging studies, and tissue biopsy:    Imaging: Chest X-ray and computed tomography (CT) scan can identify pleural thickening, pleural effusion, and other abnormalities suggestive of mesothelioma. Biopsy: Tissue biopsy, typically obtained via thoracoscopy or image-guided biopsy, is necessary to confirm the diagnosis and determine the histological subtype of mesothelioma. Pathological Examination: Microscopic examination of tissue samples by a pathologist helps differentiate mesothelioma from other lung diseases and assess tumor characteristics. Treatment Treatment of pleural mesothelioma aims to control symptoms, improve quality of life, and prolong survival: 1. Surgery: o Surgical options may include pleurectomy/decortication (removal of pleural lining) or extrapleural pneumonectomy (removal of affected lung, pleura, and nearby tissues). o Surgery is often combined with other treatments such as chemotherapy or radiation therapy. 2. Chemotherapy: o Systemic chemotherapy with drugs like pemetrexed and cisplatin is a standard treatment option for pleural mesothelioma. o Chemotherapy may be used before or after surgery to shrink tumors, control disease progression, and improve outcomes. 3. Radiation Therapy: o External beam radiation therapy may be used to target and shrink tumors, relieve symptoms such as pain or dyspnea, and improve local disease control. 4. Immunotherapy: o Immunotherapy drugs such as pembrolizumab or nivolumab may be used to stimulate the immune system and target cancer cells, particularly in advanced or recurrent disease. Prognosis The prognosis for pleural mesothelioma is generally poor, with a median survival of around 12 to 22 months from the time of diagnosis. Factors influencing prognosis include the stage of disease at diagnosis, histological subtype, patient age and overall health, and response to treatment. Early detection, multidisciplinary treatment approaches, and ongoing research into novel therapies are essential for improving outcomes and quality of life for patients with pleural mesothelioma.
