Popliteal Aneurysms: Epidemiology, Clinical Features, Investigation, and Management Popliteal aneurysms are localized dilations of the popliteal artery, which runs behind the knee. Understanding their epidemiology, clinical presentation, diagnostic evaluation, and management is crucial for effectively addressing this vascular condition. 1. Epidemiology: Popliteal aneurysms are relatively uncommon compared to aneurysms in other arterial locations, such as the aorta. They account for approximately 70-80% of peripheral artery aneurysms. Popliteal aneurysms typically occur in older adults, with a peak incidence in the sixth to eighth decades of life. Men are more commonly affected than women, with a maleto-female ratio of approximately 3:1. Risk factors for popliteal aneurysms include atherosclerosis, smoking, hypertension, and genetic predisposition. 2. Clinical Features: The clinical presentation of popliteal aneurysms can vary depending on the size of the aneurysm, its location, and the presence of complications. Common clinical features may include:     Palpable pulsatile mass behind the knee: A large popliteal aneurysm may be palpable as a pulsatile mass in the popliteal fossa. Leg or calf pain: Patients may experience discomfort or pain in the calf or leg, especially during physical activity or walking (claudication). Symptoms of acute limb ischemia: Acute thrombosis or embolization of the aneurysm can lead to sudden onset of severe pain, pallor, coldness, and loss of pulse in the affected leg. Complications: Popliteal aneurysms are prone to complications such as thrombosis, distal embolization, rupture, or compression of adjacent structures (e.g., nerves, veins). 3. Investigation: Diagnostic evaluation of popliteal aneurysms typically involves a combination of imaging studies and vascular assessment. Key investigations may include:     Duplex ultrasound: Duplex ultrasound is often used as the initial imaging modality to assess the size, location, and morphology of the aneurysm, as well as blood flow characteristics within the popliteal artery. Computed tomography angiography (CTA): CTA provides detailed anatomical information and helps visualize the extent of the aneurysm, surrounding vessels, and any associated complications. Magnetic resonance angiography (MRA): MRA is an alternative imaging modality that can provide high-resolution images of the popliteal artery and surrounding structures without the use of ionizing radiation. Angiography: Conventional angiography may be performed to delineate the arterial anatomy, identify the precise location of the aneurysm, and guide endovascular interventions. 4. Management: The management of popliteal aneurysms depends on various factors, including the size of the aneurysm, symptoms, presence of complications, and patient's overall health status. Management options may include:     Observation: Asymptomatic small popliteal aneurysms with a diameter less than 2 cm may be managed conservatively with regular surveillance and lifestyle modifications (e.g., smoking cessation, blood pressure control). Endovascular repair: Endovascular techniques, such as stent graft placement or coil embolization, can be used to exclude the aneurysm and restore normal blood flow within the popliteal artery. Endovascular repair is often preferred for small to moderate-sized aneurysms, especially in patients at high surgical risk. Surgical repair: Open surgical repair, such as aneurysmectomy with or without bypass grafting, may be indicated for large or symptomatic popliteal aneurysms, or in cases of complications such as thrombosis or rupture. Thromboembolectomy: In cases of acute limb ischemia due to thrombosis or embolization of the aneurysm, emergent thromboembolectomy may be necessary to restore blood flow and prevent limb loss.
