Compartment Syndrome: Symptoms, Signs, Diagnostic Criteria, and Management Compartment syndrome is a serious condition characterized by increased pressure within a closed anatomical compartment, leading to impaired tissue perfusion and potential tissue necrosis. Timely recognition and intervention are crucial to prevent irreversible damage and complications. Definition Compartment syndrome occurs when pressure within a muscle compartment exceeds perfusion pressure, resulting in compromised blood flow and tissue ischemia. It most commonly affects the extremities, particularly the lower leg (anterior and deep posterior compartments), but can also occur in the forearm, hand, foot, and thigh. Symptoms and Signs  Early Symptoms: o Pain: Disproportionate and intense pain that is out of proportion to the injury, often exacerbated by passive stretching of the affected muscles.  o Paresthesia: Numbness, tingling, or altered sensation in the affected area due to nerve compression. o Pain on Passive Stretch: Increased pain when the affected muscles are stretched passively. Progressive Signs: o Swelling: Visible swelling and tenseness of the affected compartment. o Pallor: Pale or blanched skin over the affected area due to reduced blood flow. o Pulselessness: Diminished or absent distal pulses may occur in severe cases, indicating compromised arterial perfusion. o Paralysis: Late sign indicating advanced nerve injury and muscle damage. Diagnostic Criteria Clinical assessment combined with intracompartmental pressure measurements are used to diagnose compartment syndrome:   Clinical Assessment: o History and physical examination focusing on pain, sensory changes, motor deficits, and signs of compartmental swelling and tightness. Intracompartmental Pressure Measurement: o Normal compartment pressure is typically less than 10-15 mmHg. Pressures exceeding 30 mmHg, especially in the presence of symptoms and signs, are highly suggestive of compartment syndrome. o Continuous monitoring may be necessary if initial measurements are inconclusive or to assess progression. Management Early recognition and prompt intervention are critical to prevent irreversible tissue damage and long-term complications: Immediate Actions:     Remove Constrictive Dressings or Splints: Relieve any external pressure exacerbating compartment syndrome. Elevate the Limb: Elevate the affected limb to heart level to reduce swelling and venous pressure. Avoid Ice: Cold therapy can worsen vasoconstriction and exacerbate ischemia. Monitor Vital Signs: Assess for signs of shock, especially in cases of acute trauma. Definitive Treatment:  Fasciotomy: Surgical decompression is the definitive treatment for compartment syndrome. o Indications: Persistent or worsening symptoms/signs despite conservative measures, intracompartmental pressures >30 mmHg, or clinical deterioration. o Procedure: Incision through the fascia to release pressure and restore circulation. o Timing: Should be performed urgently once diagnosed to prevent irreversible tissue damage. Delayed fasciotomy increases the risk of permanent muscle and nerve injury. Post-Operative Care:    Wound Care: Monitor for signs of infection and ensure adequate wound healing. Splinting and Rehabilitation: Gradual mobilization and physical therapy to regain strength and function. Monitoring: Regular assessments for compartment pressures, neurovascular status, and functional recovery. Complications and Prognosis    Muscle Necrosis: Delayed diagnosis or treatment can lead to irreversible muscle damage and compartment syndrome sequelae. Nerve Injury: Prolonged compression can cause permanent nerve damage and sensory deficits. Functional Impairment: Long-term disability may result from muscle atrophy, joint contractures, or chronic pain syndromes.
