How? •Subiective: what the client reports or others report about the client Objective: what you observe, factual info, results of screenings or assessments, info received from other treatment team members. how client responded to interventions assessment: diagnosis, Clinical impressions, presenting problem, progress on treatment goal (s) Ploun: next appointment date, items to follow up on, referrals, homework or instructions given to client Terminating Therapy: Begin with the End in Mind why? goals have been met Client is not making progress referring out for continued work change in resources (financial, time, emotional energy...) disagreement with the therapist Strategies for Terminating Therapy Professionally • Create a "termination plan" Space out final sessions (fading) identify warning signs for return of symptoms/self-monitoring normalize feelings of loss, reframe as success (graduating from therapy, growing out) plan a phone check-in 30, 60, 90 days out identify seif help groups follow-up with clients who disappear - (disappearing isn't always about you or a bad sign) it's ok to disagree remain professional Prepare for requests for casual relationships receive gifts emically and graciously
