MEDS USED FOR PRETERM LABOR/TO DELAY MAGNESIUM SULFATE Action: relaxes uterus muscles and stops contractions- OFF LABEL USE Has also been used for seizure ppx in preeclamptic and eclamptic clients Nursing implications: give through IV with a loading dose of 4-6g over 15-30mins at first, then keep infusion at 1-4g/hr Check VS and DTR (Deep tendon reflexes) hourly, report hypotension or depressed/absent DTRS Monitor LOC, HA, blurred vision, dizziness, ALOC. Keep and maintain continuous FHR monitoring Monitor I/O, report <30ml/hr output Check RR rate, report RR <12/min Check lung sounds for pulmonary edema Monitor for common maternal s/e: flushing, nausea, vomiting, dry mouth, lethargy, blurred vision, HA MONITOR FOR S/S MG TOXICITY: decreased LOC, decreased RR, decreased DTRS, slurred speech, weakness, RR/cardiac arrest ANTIDOTE: calcium gluconate ** INDOMETHACIN (INDOCIN) Actions: inhibits prostaglandins which stimulate contractions, inhibit uterus activity, arrest labor Nursing implications: check VS, uterus activity, FHR often Oral form given w food to prevent GI upset Moms with PUD should not be given this med- further GI irritation Have US to check amniotic fluid volume and function of ductus arteriosus before starting therapy Check for s/s maternal hemorrhage Monitor a/e : n/v, heartburn, rash, prolonged bleeding, oligohydramnios, HTN Neonatal a/e: constriction of ductus arteriosus, premature closure of ductus arteriosus, oligohydramnios, pulm htn Should not be given in women >32 wks NIFEDIPINE (PROCARDIA) Action: blocks calcium movement in to the muscle cells, stops uterine activity to stop preterm labor , delay progress Nursing implications: careful when giving w mg sulfate bc of increased risk of 110- report to MD Monitor for fetal effects: decreased uteroplacental blood flow , bradycardia-→ can lead to hypoxia a/e: flushing of skin, HA, transient tachycardia, palpitations. Postural hypertension, peripheral edema, transient fetal tachycardia. Should not be given to moms with cardiovascular disease of hemodynamic instability. BETAMETHASONE (CELESTONE) Actions: promotes lung maturity by stimulating surfactant productions Prevents risk of RR distress and IVH in the neonate born <34 wks Nursing implications: given 2 doses via IM 24 hrs apart Educate parents of benefits to newborn Check maternal lung sounds and monitor for s/s infection
