INTRACRANIAL ARTERIOVENOUS MALFORMATION A rare congenital disorder Caused by abnormal development of blood vessels and can occur in the brain, brain stem, or spinal cord An AVM that hemorrhages can lead to serious neurologic deficits and possibly death Other AVMS cause no problems Therapeutic management Surgical excision; endovascular embolization- closing off the vessels of the AVM by injecting glue into them Radiosurgery- involves focusing radiation on the AVM Child will usually require 24 hrs of IC monitoring s/p sx Assessment Intracranial hemorrrhage Seizures HA Progressive neurologic deficits Vision problems Loss of speech Problems w memory Paralysis In children <2 – cardiac failure, large head , seizures DX- made w MRI, CT, arteriography Nursing management Monitor for neuro changes, note seizures, s/s of increased ICP . EVD and shunt placement may be needed d/t hydrocephalus
