EFFECTS OF DRUGS ON PREGNANCY Alcohol: can lead to spontaneous abortion, insufficient weight gain. IUGR, fetal alcohol syndrome which is the leading cause of intellectual disability of NB Caffeine: vasoconstriction, mild urine output in mom, fetal stimulation Nicotine: vasoconstriction, reduced uteroplacental blood flow, < birth weight, abortion, premature birth, placenta abruption, fetal death Cocaine: vasoconstriction, gestational HTN, placental abruption. Abortion, CNS defects, IUGR Marijuana: anemia, low weight gain, hyperactive reflexes (startle reflex), newborn tremors, premature, IUGR Opiates/Narcotics: maternal and fetal withdrawal, placental abruptions, PROM, preterm labor, perinatal asphyxia, newborn sepsis, death, intellectual impairments, malnutrition, LBW Sedatives: CNS depression, newborn withdrawal, maternal seizures, neonatal abstinence syndrome, delayed lung maturity. URINE TOXICOLOGY- can be helpful to determine drug use. Length of time a drug presents in urine is: Cocaine: 24-48 hrs in adult, 72-96hrs in infant Heroin: 24hrs in adult, 24-48hrs in infant Opioids: 1-4 days after use Marijuana: 1wk-1month in adult, 1+ month in infant Methadone: 10 days in infant
