Comprehensive Review of Fracture types ● Fracture = structural discontinuity in bone ● Biomechanical failure of bone materials ○ When applied stress exceeds the maximum strength hold possible ● High force trauma ● Normal/low force trauma ○ Lower force for fracture to happen ○ Pathologically weaken the bone ● Repetitive cyclic loading ○ Leading microcrack accumulation ○ Outspace repair Universal classification of system ● Fractures are described by systemic approach ● This is to universally inform prognosis and treatment ● Based on relationship on external environments ○ Closed fracture ■ Fracture that does not extends to external environments ■ Soft tissue over fractures ■ Lower risk in osteomyelitis ■ Tx focused on fracture ○ Open fracture ■ Haematoma extends to external environment ■ Breach in skin and soft tissues ■ High risk in infection ● Osteomyelitis ■ Non- union ● Gusto-Anderson Classifications ○ Type 1 ■ Wound <1cm ■ Clean ■ Menial soft tissue damage ■ Simple fractures ○ Type 2 ■ Wound >1cm ■ Moderate soft tissue damage ■ Less contaminations ■ Simple fractures ○ Type 3 ■ High E trauma ■ Large soft tissue damage ■ Significant contaminations ● IIIA ○ Adequate soft tissue coverage of bone ○ Extensive oft tissue laceration ● IIIB ○ Inadequate soft tissue coverage ○ Periosteal stripping ○ Need for flap coverage ● IIIC ○ Any type of open fracture ○ a/w arterial injury ● Anatomical Displacement background ○ Non-displaced ■ Fragment maintain in anatomical sound positions ○ Displaced ■ Fragments have lost in alignment positions ● Shift ○ medial , lateral, anterior, posterior ● Tilt ○ Varus, valgus, apexanterior/posterior ● Shift ○ One fragment to another ● Rotations ○ Twisting on long axis
