Tinnitus NICE guidelines ● Frist contact ○ Must know person’s experience in tinnitus ○ Impact ○ Concern ○ Preference for their Mx ● Make a Mx plan ⇒with family/supporters relevant ● Must provide information of tinnitus ○ What is it? ○ Possible causes ○ What could be worsen ■ Loud noise, stress ○ Safe listening practices ● Must assure that tinnitus is common ○ Not a sign of critical disease ○ Has high success rates in Mx Ix ● Can ask about the tinnitus functional index ● Could also know it by asking other measures ○ Visual-analogue scale ○ Possible if question not feasible ● Ask on quality of life ○ Effecting patients in sleep, mental health, daily functionings ○ Audiological assessments ● DO NOT ROUTINELY OFFER ○ Psychoacoustic ○ tests ■ pitch/loudness matching ○ Otoacoutis emission test ○ Acoustic reflex ■ Testing for tinnitus ● Imaging ○ Non-pulstile tinnitus on neurological /otological / head and neck signs ⇒ MRI ■ When MRI is not possible ⇒ contrast enhanced CT ○ Imaging in unilateral and asymmetric could be also an option to think about in non-pulsatile tinnitus ■ Even when there arn’t any signs ■ This is because there could be underlying pathology ○ You should NOT offer imaging in symmetrical and non-pulsatile tinnitus if there are no other signs Mx ● When patient have significant hearing loss ○ Amplification device could be considered ○ + once it is in condition of losing ability of communications ● Sound therapy ○ Evidence not strong enough for recommendations ○ NICE notes it needs more research on it ● Psychology therapies ○ Tinnitus related distress ■ Tinnitus impact on emotional specific CBT ■ Group based CBT ■ Acceptance commitment therapy ● Support ○ Ongoing discussion ○ Plan over time
