APNEA- PEDS Absence of breathing for longer than 20secs- can also be accompanied by bradycardia Can present as (ALTE) – acute life threatening event → event in which infant has combination of apnea, color change, muscle tone alteration, coughing, or gagging Can also occur d/t RR distress Apnea in NB can be associated w hypothermia, hypoglycemia, infection or hyperbilirubinemia Can also occur in prematurity d/t immature respiratory system Should NOT be considered a predecessor to SIDS When it occurs d/t another disorder or infection- tx is directed toward the cause Stimulation can trigger baby to take a breath- if not effective, rescue breathing or bag valve mask ventilation is necessary Caffeine or theophylline is sometimes given in premature babies to stimulate respirations * Nursing assessment: Absence of respirations Position Color Emesis on bedclothes Pulselessness – if apnea doesn’t resolve when stimulated Nursing management : Stimulate gently Maintain neutral thermal environment Caffeine or theophylline if prescribed If recurrent ALTE- infant can be dc home on a home apnea monitor → the monitor is usually discontinued after 3 months without a significant event of apnea or bradycardia
