CRANIOSYNOSTOSIS – PEDS Premature closure of the cranial sutures – which shouldn’t normally occur until LATE CHILDHOOD Premature closure can inhibit brain growth and a distorted skull appearance will show If only 1 suture is fused- neuro impairments are rarely seen 2 or more sutures fused- neuro complications such as hydrocephalus w increased ICP more likely to occur Cause is unknown Prognosis is good for the majority of infants and normal brain development usually occurs → EXCEPTIONS ; infant or child who has an associated genetic d/o that involves brain function and development Sx correction can be done to allow for normal expansion of the brain and to get a normal appearance of the head/skull Assessment Most cases present at birth Skull deformity can be see Prominent bony ridge that can be palpated XRAY confirms FUSION OF SUTURES Measure head circumference in all children <3 yrs old and compare findings w normal head circumference parameters – and past measurements of that child ** Nursing management Focus on post op care Observe H &H levels d/t L volumes of blood loss Observe for pain Observe for hemorrhage, fever, infection, swelling Large amts of facial swelling can present d/t location of the sx and incision line – child might be unable to open eyes for a few days post op ; provide parent teaching regarding this.
