LEGG CALVE PERTHES DISEASE- PEDS Self limiting condition ** - NO TX NEEDED Involves avascular necrosis of the femoral head Most affected are BOYS 4-8 yr old Unknown eitiology but interruption of blood supply to femoral head results in bone death and spherical shape of the femoral head is lost Swelling of soft tissue around hip happens When new blood vessels develop the area is supplied w circulation, bone resorption occurs and deposition takes place. Revascularization takes 18-24mo and during this time, bone is at INCREASED R/O FRACTURES bc its soft. Takes time- but the femoral head does reform Therapeutic management Goal is to keep normal femoral head shape to restore motion Anti-inflammatory meds to < muscle spasms + relieve pain Limit activity Bracing Casting Traction Serial XRAY f/u to determine progress If sx warranted- osteotomy can be performed Assessment Short stature Family history Painless limp- intermittent or over many months Mild hip pain Pain when exercising Trendelenburg gait During ROM- internal rotation of hip and limited abduction Muscle spasms Nursing management: Anti-inflammatory meds Exercise unaffected body parts Brace application (provide family teaching if needed) Avoid high contact sports- recommend swimming or bicycle riding
