Malrotation and volvulus Intestinal malrotation - results from a disruption in embryonic development ↓ The intestine is abnormally attached and the mesentery narrows, twisting on itself (volvulus) ↓ If the volvulus involves the entire small bowel, it’s called midgut volvulus * Main s/s: bilious vomiting abdominal pain, shock symptoms, abdominal distention, tachycardia, bloody stools Therapeutic management: surgery; LADD procedure ⤷ The intestine is straightened out and bands contributing to the misalignment are divided. If necrosis has occurred – ostomy may be needed. Assessment: Vomiting Abdominal pain Hypoactive bowel sounds Abdominal guarding or rebound tenderness KUB (Kidney, Ureters, Bladder) - can reveal obstruction Upper GI series - will show corkscrew appearance of the twisted bowel Nursing Management: IVF (Intravenous Fluids) IV ABX (Intravenous Antibiotics) NGT (Nasogastric Tube) to decompress the stomach Prepare for surgery Post-op care
