PERINATAL ASPHYXIA At RISK neonates SGA (common)- poor tolerance to stress of labor frequently leading to acidosis and hypoxia Living in hypoxic environment prior to birth due to placental insufficiency, leaves little to no O2 reserves that can withstand stress of labor Contractions > hypoxic stress Depletion of glycogen Impaired placental circulation Post term newborns- placental deprivation or oligohydramnios leading to cord compression and subsequent reduction in perfusion to the uterus Preterm newborns (common)- lack of surfactant, unstable chest wall, immature lungs and respiratory control centers in the CNS Small RR passages → > obstruction Unable to clear mucus from small RR passages Assessment: Fetal distress- bradycardia, and decels during labor Low APGAR scores There could be meconium in the amniotic fluid Interventions: anticipate a problem Assess maternal risk factors Start resuscitation measures immediately after birth
