TTN (transient tachypnea of the newborn) Self limiting condition that involves mild RR distress Results due to delay in clearance of fetal lung liquid that leads to pulm edema Usually occurs within a few hrs of birth and resu=olves 48-72 hrs after Occurs in 1% of births Higher risk in males During fetal life- lungs fill w fluid bc the placenta is used for nutrient and gas exchange → at birth this fluid must be removed and replaced w air. CSECTION babies are at risk of excess fluid d/t not going through all stages of labor- passage through the vagina during labor compresses the thorax and helps remove this fluid- CSECTION deprives baby from this compression. FLUID REMAINS THERE. So TTN occurs when the liquid is removed slowly or incompletely Excess fluid results in workload Assessment: s/s last more than 72hrs tachypnea expiratory grunting mild intercostal retractions < breath sounds Labored breathing Nasal flaring Crackles Mild cyanosis RISK FACTORS: moms who have been heavily sedated, gestational diabetes, newborns born via Csection, prolonged labor, fetal macrosomia, breeched births, rapid labor/birth, infants w hypothermia, infants born<38 wks, maternal asthma, and smoking. Lab and dx : CXR, lung US will show mild symmetric lung hyperaeriation and prominent perihilar interstitial markings Nursing management: provide adequate oxygenation IVF or gavage feedings until the RR < Keep neutral thermal environment with minimal stimulation to minimize oxygen demand Provide ongoing assessment to monitor for improving condition
