MEDS USED FOR CERVICAL RIPENING AND LABOR INDUCTION DINOPROSTONE Action: softens and dilates the cervix to ripen and INDUCE labor It’s the only drug FDA approved for this Nursing implications: provide emotional support Given pain meds if needed Assess dilation and effacement often Monitor for contractions Assess mom and babys FHR frequently S/E: HA, nausea, vomiting, diarrhea MISOPROSTOL (CYTOTEC) Action: ripens cervix to induce labor Nursing implications: instruct client about purpose and possible a/e Needs informed consent* Assess VS and FHR often Initiate OXYTOCIN for labor induction at least 4 hrs after last dose is administered. S/E : nausea, vomiting, diarrhea, uterine hyperstimulation, and category II FHR patterns OXYTOCIN (PITOCIN) Action: acts on uterine myofibrils to contract to initiate or reinforce labor Nursing implications: give it as IV with PUMP Increase dose based on protocol until adequate labor progress is achieved Assess moms VS and babys FHR – BASELINE and after starting the oxytocin infusion Determine frequency, duration, and strength of contractions frequently Let MD know if there is uterine hypertonicity or abnormal FHR patterns Keep I/O- watch for water intoxication S/E: hyperstimulation of uterus, impaired uterine blood flow leading to fetus hypoxia, rapid labor leading to cervical lacerations, uterine rupture, hypotension
